Sample workplace assessment

Fictional example — not a real client assessment.

All workplaces, observations, roles, priorities and tracker entries below are invented for illustration. This sample does not assess your workplace, certify legal compliance or establish safe conditions. Four example entries do not imply a guaranteed hazard count.

01 · Scope & context

Example Distribution Warehouse — fictional workplace

Illustrative visual review of a receiving and shipping area, designated pedestrian routes, fire protection equipment, machine guarding and selected training records. No equipment testing, engineering review, comprehensive fire-code inspection or regulatory compliance audit.

A real engagement records the actual review scope, observed conditions, evidence and limitations. Scope, fees, deliverables and turnaround are agreed with the client in the proposal. Photos are included only where appropriate and permitted. Generated visuals, if shown, are illustrative and not documentary evidence.

02 · High-priority sample findings & actions

Fire protection, machine safety and warehouse hazards.

This fictional example includes a suspected low-pressure fire extinguisher, an obstructed pedestrian route near forklift movement, and an exposed conveyor nip point. Priorities below are illustrative, not a universal scoring system. A real assessment considers exposure, potential severity, existing controls and verification needs. Actual findings are reported without padding, even if few or no deficiencies are identified.

EX-01 · Fire protection — extinguisher pressure

Illustrative priority: High — prompt action

Example observation
During this fictional walkthrough, the needle on a wall-mounted portable fire extinguisher pressure gauge appears below the manufacturer's indicated operating range. The unit is identified for immediate verification rather than presumed ready for emergency use.
Priority rationale
An extinguisher that is undercharged or otherwise unserviceable may fail to provide effective first-response firefighting capability. Confirm the exact gauge indication, type and service status with a competent provider.
Recommended action
Notify site management; remove the suspect unit from service following site procedure, arrange inspection/servicing or replacement by a qualified provider, and confirm suitable fire-extinguisher coverage remains available. Document corrective action and verification.

EX-02 · Forklift and pedestrian traffic

Illustrative priority: High — prompt action

Example observation
A shrink-wrapped pallet is staged partly inside a designated pedestrian walkway while powered industrial equipment operates in an adjacent receiving aisle.
Priority rationale
Reduced pedestrian clearance and separation can increase the possibility of workers entering vehicle travel paths and being struck.
Recommended action
Remove the pallet obstruction; maintain marked pedestrian routes; verify traffic separation and staging boundaries; brief affected personnel and inspect the route again.

EX-03 · Machine guarding — exposed moving parts

Illustrative priority: High — stop and isolate affected equipment

Example observation
An example conveyor drive at the packaging line is shown operating with an accessible nip point because a fixed guard is missing from the normal operating position.
Priority rationale
Access to moving transmission components or nip points may cause entanglement or crushing injury. Actual guarding requirements depend on the machine and applicable standards.
Recommended action
Stop affected equipment and prevent further operation until hazards are evaluated and appropriate guarding is restored by authorized personnel. Follow applicable lockout and verification procedures before servicing; confirm safe condition before restart.

EX-04 · Training record verification

Illustrative priority: Verification needed

Example observation
The selected fictional record set does not include a retrievable entry for a worker's site-specific instruction.
Priority rationale
A missing document alone does not prove instruction was not provided. Verify actual training and demonstrated competency before assigning a finding.
Recommended action
Check alternate records and interview relevant supervisors or workers. Complete any verified outstanding instruction and document the result.

03 · Illustrative action tracker

Assign. Track. Review.

Fictional owners, targets and statuses only; not SENVRIC delivery commitments.

Follow-through & limitations

FindThree includes one complimentary 30-minute virtual follow-up within 30 days of report delivery to clarify recommendations and review progress. Site revisits, implementation and ongoing support are quoted separately. If the report omits an agreed deliverable or does not address the agreed scope, SENVRIC corrects that shortcoming at no additional fee.

SENVRIC's work is advisory, not legal advice or regulatory certification. Findings reflect the agreed scope and observed conditions, not a guarantee of legal compliance, hazard elimination or financial savings. Employers retain their legal obligations. Existing engagements remain subject to their separately agreed terms.
